Eye-Opening Effect Achieved by Modified Transconjunctival Lower Blepharoplasty

Summary: In this study, performed in Japan, 40 patients with sunken eyes and drooping eyelids underwent a modified transconjunctival lower blepharoplasty, enhanced by “deframing” and targeted decompression of the lower orbital fat compartments, to improve both lower and upper lid appearance. Using 3D imaging, researchers measured the distance between the eyelids (palpebral fissure height) before surgery and again 12 months later, finding a significant average increase from 8.41 mm to about 9.24 mm on each side. On average, 0.43 g of fat was removed per eye, with the largest amount taken from the lateral fat compartment. These results demonstrate that this procedure not only opens the eyes more effectively but also provides lasting relief of other periorbital symptoms.
